THE CHALLENGE

Following hospital discharge, individuals with complex cardiovascular disorders face intensive healthcare management routines at home. Because unexpected physiological shifts act as early indicators of advancing heart failure, tracking daily fluctuations in weight, activity, and fluid retention becomes critical.

To solve this, Alfred Health needed a digital system designed to:

  • Convert intricate, hospital-validated monitoring methodologies into a user-friendly mobile application interface.
  • Transmit critical biometric records securely to a clinical dashboard, eliminating additional administrative overhead for practitioners.
  • Adhere to strict international data privacy and Information Security standards, meeting the demanding compliance criteria of hospital governance and HIPAA/FHIR frameworks.

THE SOLUTION

To address the “continuity of care” gap in managing chronic heart failure, where traditional, periodic appointments fall short, Hitori collaborated with the Alfred Hospital Heart Failure team and Apple Health specialists. Together, they engineered pump, a secure, high-performance digital health ecosystem consisting of a native iOS companion application for patients alongside a cloud-based web portal for clinicians.

This dual-portal architecture functions as a secure, real-time bridge connecting the home to the clinic. By utilising Apple HealthKit to integrate biometrics directly from the user’s device, the platform effectively converts passive data into active clinical insights. This empowers multidisciplinary medical teams to track patient vitals remotely, predict adverse events, and intervene early, driving superior recovery tracking and heightened patient engagement.

The Patient Companion App (Native iOS)

Serving as a dedicated health companion, the mobile application is fittingly named pump. The app is engineered natively in Swift to deliver speed and optimal performance, guiding patients through daily check-ins modeled after the Alfred Cardiology team’s specialised clinical methods.

Through deep integration with the Apple HealthKit ecosystem, the application passively collects background steps and activity metrics. Over time, this builds an objective baseline of patient mobility and physical tolerance.
